PA State Health Requirements

Physical Exams

All students in Kindergarten, 6th and 11th grades are required to have a physical exam completed and the results on file with the nursing department NO LATER than April30th, 2024. Physical exams from July 1st of 2022 until April 30th of 2024 will be accepted. Forms are available in Focus, or from the school nurse responsible for your child. Physician generated forms are also acceptable.


Dental Exams

All students in Kindergarten, 3rd and 7th grades are required to have a dental exam completed and the results on file with the nursing department NO LATER than April 30th, 2024. Dental exams from July 1st of 2022 until April 30th of 2024 will be accepted. Forms are available in Focus, or from the school nurse responsible for your child. Dentist generated forms are also acceptable.


Health Screenings

All students in All grades are required to be screened for height, weight and vision. All students in grades K-3, 7 and 11 are required to be screened for hearing, and All students in grades 6 and 7 are required to be screened for scoliosis. Due to the nature of our school, it is not possible for the nursing department to screen All students. We ask that if your child has a physician appointment from July 1st,2023 until April 30th, 2024, you have the required screenings completed and the results sent to the nursing department. The screening form is available from the school nurse responsible for your child or in Focus.


Immunizations

All students in 7th grade are required to have a dose of Tdap and MCV4(meningitis). All students in 12th grade are required to have a second dose of MCV4(meningitis).The nursing department must have proof of these immunizations on file NO LATER than September 30th, 2023. If your student currently has an exemption on file, then no updated immunization record is needed. If you are unable to meet this requirement by the September 30th date, you MUST reach out to the school nurse responsible for your student IN ADVANCE of this date to discuss other options. It is the responsibility of the parent/guardian to provide an updated record to the school nurse.
